The company's WellFit Program reimburses employees, and/or their spouses, 50% of the monthly fee (not to exceed $25/month) to an approved fitness facility or fitness class.
Who's eligible to receive the incentive?
All regular full-time and regular part-time, benefits eligible, employees, and/or spouses, who join approved fitness facilities or attend a fitness class, are eligible.
What fitness classes are eligible for reimbursement?
Fitness classes that are taught by certified aerobic instructors. Suggested classes include step aerobics, jazzercise, kickboxing, yoga, pilates, tae kwon do, etc.
How much will the company pay?
The company's will reimburse you for 50% of the monthly facility fee, up to a maximum of $25 a month or $75 per quarter. If you pay your fee annually, the company will reimburse you for half that fee, up to $300. Initial membership fees or equipment costs are not eligible for reimbursement. Fitness classes are reimbursed 50% of the cost up to $300 annually.
Will the company pay the incentive for a family membership to an approved fitness facility?
Yes. Family memberships at approved fitness facilities that allow dependents to use the facility are reimbursable; however, the membership must be in the employee's or spouse's name.
Will the company pay the incentive for a dependent's facility membership or classes?
No. Only employees and their spouses are eligible for the reimbursement benefit.
Can both my spouse and I receive the reimbursement incentive?
Yes. If you join different facilities, you each will be eligible for the reimbursement based upon an individual membership. Only one family membership per household is eligible for reimbursement. If you both attend fitness classes, you will both be eligible for reimbursement.
If my spouse and I are both employees, do we receive double reimbursement?
If you join the same fitness facility with a family membership, only one of you will be eligible for reimbursement. However, if you join different facilities, you each will be eligible for the reimbursement based on an individual membership. If you both attend fitness classes, you will both be eligible for reimbursement.
Do I have to fulfill any guidelines to receive the incentive?
You are expected to actively participate in a fitness program a minimum of 10 times a month to ensure that you receive the maximum physical benefit.
To whom do I pay my membership fee?
Make payments directly to the fitness facility or fitness class.
How do I apply for the fitness center/class incentive?
- Join an approved facility on the approved fitness facility list or a fitness class.
- Submit your request for reimbursement through the automated Wellfit Reimbursement system in PeopleSoft (My HR) on the company's Intranet.
Is the reimbursement considered taxable income?
Yes. According to IRS regulations, reimbursements are considered taxable income.
How often will the reimbursement be paid?
Reimbursements will be included as earnings on the paycheck following request for reimbursement and approval through the Wellfit Reimbursement system. After approval, an e-mail is generated and sent to confirm the paycheck on which earnings will be applied.
Can I file for reimbursement at any time during the year?
You can submit requests for reimbursement at any time; however, you must request reimbursement by April 5 for the previous calendar year (for example, fees paid in 2003 must be submitted by April 5, 2004).
