An easy call.

Do you have high blood pressure, high cholesterol, excess weight or insulin resistance? If so, call Anthem's free and confidential Condition Care Management* program to take control of your health. Follow these steps to get started:

1. Call 1-866-760-7537. A recording will ask the following questions:
   
   a. Which program are you calling about? Say "Condition Care Management."
   
   b. Do you know the extension of the person you are calling? Say "No."
   
   c. Are you calling to enroll in one of our Condition Management Programs? Say "Yes."
   
   d. Your call will be connected automatically to a live Anthem representative who will ask you questions to register you in the correct program.
   
   
2. Identify yourself as an Anthem participant. Tell the representative which health condition you have (high blood pressure, high cholesterol, excess weight, insulin resistance, etc).
3. Tell the representative you want to enroll in Condition Care Management.
